Medicine Accreditation Statement
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the University of Kentucky College of Medicine and ACCC. The University Of Kentucky College Of Medicine is accredited by the ACCME to provide continuing medical education for physicians.
The University of Kentucky, College of Medicine designates this educational activity for a maximum of 15.5 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
The University of Kentucky College of Medicine presents this activity for educational purposes only. Participants are expected to utilize their own expertise and judgment while engaged in the practice of medicine. The content of the presentations is provided solely by presenters who have been selected for presentations because of recognized expertise in their field.
Nursing Accreditation Statement
The University of Kentucky, College of Nursing is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.
The Kentucky Board of Nursing approves the College of Nursing as a continuing nursing educa-tion provider. Kentucky Board of Nursing approval of an individual nursing continuing education provider does not constitute endorsement of program content. This activity is provided for a maximum of ANCC (60 minute) 15.5 and KBN (50 minute) 18.5 contact hours for nurses who attend all sessions (9/18/08 = KBN 6.9[ANCC = 5.75] ), ( 9/19/08 = KBN 8.0 [ANCC 6.75]) and 9/20/08 = KBN 3.6[ANCC 3.0] contact hours, Provider #: 3-0008-01-2013-062-063-064. A certificate will be issued after the conference.
Pharmacy Accreditation Statement
The University of Kentucky College of Pharmacy is approved by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy eduocation. This program will award 15 contact hours (1.5 CEUs) of continuing pharmacy education credit in states that recognize ACPE providers. Statements of credit will indicate hours and CEUs based on participation and will be issued at the conclusion of the activity. The college complies with the Criteria for Quality for continuing education pro-gramming.
Other Continuing Education Certificates
The University of Kentucky will provide non-physicians, RPh, etc. with certificates of hours of participation. Each individual should claim only those hours actually spent in the educational activity.
