Care Coordination Model Thumbnail

To leverage assessment results for the development of quality improvement (QI) projects, the Model Full Report is an ideal starting place. In the report, the Model’s 12 Assessment Areas map to more than 100 quality measures (see pages 20-37) for which cancer programs are likely already collecting and reporting data. For example, measures include those from the Centers for Medicare & Medicaid Services Merit-based Incentive Payment System (CMS MIPS), the American Society of Clinical Oncology Quality Oncology Practice Initiative (ASCO QOPI), the National Committee for Quality Assurance Patient-Centered Specialty Practice Recognition (NCQA PCSP), the Agency for Healthcare Research and Quality Consumer Assessment of Healthcare Providers and Systems (AHRQ CAHPS®), and the Commission on Cancer (CoC) 2020 Cancer Program Standards.

Below find ACCC-curated resources to help make the case for developing and implementing a QI project for patients with lung cancer in the areas of Care Coordination and Access, Disparities in Care, and Quality Measurement and Improvement.
