Shaw at Home: An Oncology Specific Home and Community-Based Palliative Care Program

This program expands palliative care services beyond the hospital and clinic setting to provide comprehensive, in-home emotional, spiritual, practical, financial, and physical support for oncology patients. Since its inception, Shaw at Home has served over 123 unique patients, providing 1,427 palliative care visits that include goals of care discussions, symptom management, and psychological support, improving patient outcomes in rural communities.
