Implementing an Actionable Framework to Support Equitable and Comprehensive Care

Despite significant progress, cancer continues to pose a major health challenge. This is especially true for certain groups in the United States who continue to experience a disproportionate burden of cancer.

ACCC, in partnership with the Center for Business Models in Healthcare, is making 4R Care Sequences® available at no cost to ACCC Members.

4R Care Sequences® are guideline-based care plans for providers and patients, at diagnosis and at transitions between treatments.

  • 1-page templates that are personalized for specific patient populations at each point in case, available in hard copy or electronically
  • Cancer Care Sequence® plans for breast, lung, and other cancers
  • Integrated multidisciplinary cancer care including supportive care and social support
  • Tested in 30 diverse settings and populations: safety-net, VA, community, academic, rural
  • Available in English, Spanish, and other languages
  • Supports care management from initiation (workup) through survivorship
  • May be used as part of a quality improvement project supporting Commission on Cancer (CoC) accreditation QI requirement
