American Society of Clinical Oncology
Cancer Survivorship compendium of tools for oncology providers to implement or improve survivorship care in their practices. Focused on adults who have completed curative treatment or who have transitioned to maintenance or prophylactic therapy. ASCO also has a “Providing High Quality Survivorship Care in Practice” guide and evidence-based clinical practice guidelines for survivors of adult-onset cancers, and a member online survivorship care forum.

National Coalition for Cancer Survivorship
NCCS resources for providers include a cancer survival toolbox, support for planning patient care, and a toolbox facilitator manual – a training handbook to help professionals use the toolbox in interactive groups for teaching self-advocacy skills.
