Liver Cancer

Early diagnosis of HCC can be cured but lack of awareness prevents some patients from receiving curative therapy. This education series will focus on treatment approaches for HCC that can potentially eliminate disparities in access to care and review care coordination strategies to optimize patient outcomes.

Because community practitioners do not specialize in one tumor type, it can be difficult to stay up to date on safety and efficacy data for targeted therapies. To help fill this gap, including nurse navigators in community cancer settings is an important addition to overcoming disparities in treatments because they improve patient outreach, coordinate care within a multidisciplinary team, and help patients and caretakers navigate the healthcare system.

Immunotherapies that can be administered in a community oncology setting are available, but unawareness of these therapies leads to the disparities in minority groups. Thus, provider education is increasingly important for equitable access to quality care for all HCC patients. Furthermore, multidisciplinary clinics have improved equity in treatment access, but barriers still exist in provider education and patient management such as variable physician attendance and need for an intensive referral infrastructure.

Multidisciplinary HCC Care Effective Practice Guide
This report highlights effective practices in care coordination in action and provides valuable insights for cancer programs to consider. Findings from an environmental scan helped inform focus groups and interviews with three ACCC Member Programs exploring their effective practices in HCC care coordination.
