Age-Friendly Health Systems Action Community Informational Call

Wed, June 22, 2022
1:00 PM to 2:00 PM (Eastern Daylight Time)

Becoming an Age-Friendly Health System* entails reliably providing a set of specific, evidence-based geriatric best practice interventions to all older adults in your health system, known as the 4Ms — What Matters, Medication, Mentation, and Mobility — which make the complex care of older adults more manageable.

Join Us: Age-Friendly Health Systems Action Community

An Action Community is a network of teams from across different health systems who come together to accelerate their own adoption of the 4Ms. Guided by expert faculty and an “all teach all learn model,” teams participate on monthly webinars, attend convening, and test specific changes to improve care for older adults.

To learn about the Action Community, join an informational call on Wednesday, June 22 at 1:00 PM EDT.
